The following memo will assist you in completing pre-authorization requirements with your health insurance provider. It is usually beneficial if a family physician or a counselor provides the referral and is able to show medical necessity for our services.

RE: Marble Retreat Intensive Outpatient Counseling Program
Marble Retreat is a brief intensive outpatient counseling program for clergy and individuals in ministry. Marble Retreat provides services in an office setting and is in no way a hospital-based facility. In addition to group and individual counseling hours billed (detailed below) patients are billed for guest services (code 90899) which includes room and board in the amount of $730.00 per person. This 8-day provision is necessary as our patients travel from various areas of the United States to participate in the Marble Retreat program. Marble Retreat was so named to enable those in ministry and general Christian practice in need of psychiatric or psychological care to feel less threatened and escape the stigma of going to a mental health professional. Many church groups are still resistant to accepting psychiatry or counseling as a useful treatment for those in ministry or living a Christian life. Going to a "Retreat" is more acceptable. The program is patterned after the Intensive Psychotherapy Center at the Mayo Clinic, Rochester, MN, with 9 three-hour sessions of group counseling (Monday-Friday for 8 days) and three 60 minute sessions of individual counseling per person over the 8-day program.
Marble Retreat was founded in 1974 and is co-directed by Steven A. Cappa, Psy. D. and Patricia A. Cappa, L.M.F.T., CAC II. All staff therapists are professionally trained, licensed professionals or license eligible professionals.
Our counseling charges currently are $60.00 per hour for group counseling (code 90853) and $150.00 per hour for individual sessions (code 90806) for a total of $2,070.00 per person. Total counseling hours over the 8-day program are 27 hours of group and 3 hours of individual counseling.
